This form may be used to appeal a City decision on a Site Plan application. An appeal of a Site Plan decision is limited to instances where the aggrieved party alleges there was an error in a decision or determination in the enforcement of the Zoning Ordinance or applicable regulatory requirements. Decisions by the City may be appealed by the Applicant of the site plan, or any City of Peoria property owner or property owners within the specified notification area identified within the Zoning Ordinance.

The appellant may designate someone to act as their representative during the appeal process. This person (or “Representative”) shall attend all applicable public hearings or meetings on behalf of the appellant, along with provide all information and communications from the City to the appellant.
